Death Certificates Additional Information

INFORMATION REGARDING DEATH CERTIFICATES IN THE STATE OF CONNECTICUT

The Town Clerk’s Office is also the Registrar of Vital Statistics Office in the Town of Pomfret.  The original death certificate/record of any person who died in the Town of Pomfret would be on file in the Pomfret Town Clerk’s Office.  Certified copies from other towns in the State of Connecticut for persons that were living in Pomfret at the time they died would also be on file in the Pomfret Town Clerk’s Office.

Connecticut General State Statute 7-51a states that any person eighteen years of age or older may purchase certified copies of a specific death record.  However, the social security number or administrative purposes section will not be included unless the certificate is being issued to a surviving spouse or next of kin (or others specifically approved by the Connecticut General State Statutes and the Department of Public Health).

To request a certified copy of a death certificate/record in the Town of Pomfret:

Fill out the attached Request for a Certified Copy of a Death Certificate form completely.  Submit the completed form, a Postal Money Order, bank cashier’s check, or bank money order for the appropriate amount, a self-addressed postage paid envelope, and mail to:  Pomfret Town Clerk’s Office, Town of Pomfret, Five Haven Road, Pomfret Center, CT 06259
