Additional Information / Instructions for Marriage License:

• Licenses are only good for 65 days.
• You must be getting married in Pomfret to get your marriage license in Pomfret.
• Groom info on left, bride info on right.
• Use complete names (first, middle & last) for bride/groom & all parents
• For Mothers Maiden name please provide her full name (first, middle & maiden)
• Use city, as well as state for parents places of birth, if known
• Need date of marriage on top of form.
• Need contact phone # for one or both of applicants as well as the officiator.
• Please print neatly as the license is typed from this worksheet.
